The Mount Sinai Academic Family Health Team provides comprehensive, patient-centred care throughout every stage of life—from pregnancy and childhood through adulthood and older age. We focus not only on treating illness, but on prevention, early detection, health promotion and building lasting relationships with patients and families.
Our interdisciplinary team of family physicians, nurses, social workers, dietitians and other health-care professionals addresses the physical, emotional and social needs of our patients—from acute and chronic conditions to mental health, nutrition, reproductive health, diabetes, cardiovascular risk and healthy aging.
As an academic Family Health Team, our impact extends far beyond the patients we serve. We educate and mentor the next generation of family physicians and health-care professionals who will care for communities across Canada.
